Constance is a daily check-in service that helps seniors maintain their independence while giving loved ones peace of mind.
Here’s how we do it:

- We check in daily: Your Constance Family Coordinator calls your loved one, and they talk about what’s new, plans for the day, and how your loved one is feeling physically and emotionally.

- We send you a report: After talking with your loved one, your Coordinator updates you on their mood, meals, medications, appointments, and more.

- We provide consistency, coordination and confidence: Your Coordinator gets to know your loved one. Our relationship helps us recognize changes and identify needs.

Constance also includes private messaging between caregivers, family members, and the Constance team. If you have special requests or questions, just message us right inside the app.

Constance never diagnoses or treats patients. Data is stored on a secure, HIPAA-compliant platform, and users determine how their information is shared.
