The Heart for Health application is available by invitation only.

The Heart for Health application allows you to easily monitor your health and lifestyle, and safely communicate your health measurements with your own doctor. You and your doctor are supported in choosing the best diagnostics, treatment and monitoring plan without requiring a face to face appointment.

With daily and weekly measurements, you can identify trends in your behavior which help to improve your personal health.
Your doctor can make an informed and personalized treatment plan, including any medication and lifestyle advice, based on closely monitoring your health measurements over time. The Heart for Health application can only be used if your doctor has prescribed it to you. The following steps need to be taken to get started:

1. When visiting your doctor, the Heart for Health application is prescribed to you
2. You will receive an e-mail with instructions on how to download, install and connect the application
3. You can start measuring the blood pressure values
4. You can fill in answers to questionnaires about your health status
5. Your physician will monitor your measurements and answers and make informed decisions about your treatment with support from scientific guidelines.
