Huddle for Care 1.0

Huddle for Care is a web-based platform that fosters online sharing and learning between professionals working to strengthen care transitions. Our goal is to accelerate the spread of ideas across the country by creating a forum in which transitional care implementers can share their stories and tools they found effective in overcoming challenges and meeting goals. Huddle aims to provide practical, tangible strategies that a nurse, case manager, discharge planner or any other care transitions professional can pick up and test in their organization, all while connecting and problem-solving within a community of their peers from around the country.
